Mountain View Medical Center, New PatientGetting ready to see a new provider can be a little scary, it shouldn’t be!

We want to make your first visit as stress-free as possible. We pride ourselves on keeping wait times down, however, there are some things you will want to make sure to bring at the time of your appointment to limit your waiting time even further.

  • Your insurance card
  • A valid photo ID
  • New patient paperwork

Your new patient paperwork will provide us with information pertaining to your medical history and the reason for you visit. In addition to this we also request an authorization form in order to obtain your medical records from your previous providers.

We have all these form available on our website (links below) or you can request them by mail at the time you make your appointment.

Additionally, prepare a list of questions you want to ask your provider. You don’t wan to leave your appointment then later remember what you wanted to ask – if this does happen, don’t be afraid to give the office a call, our staff can have your provider give you a call when time allows.

Lastly, by far the most important thing you can do during your appointment is to be truthful. Let your provider know everything that’s going on with you. If you smoke, drink, use drugs, or engage in other risky behaviors, let them know. Remember that your conversation is private and privileged. If you haven’t been good about remembering your medication, let them know. It is vital to your health for your provider to be fully aware of anything medical related.
