As a Mountain View Medical Center Patient, you have the Right To:

  • Be treated with dignity and respect
  • Receive care free from discrimination based on race, ethnicity, gender, religion, national origin, sexual orientation, disability, or age
  • Be fully informed about your treatment, with up-to-date information
  • Expect your medical and personal information to remain private
  • Participate actively in creating your care plan
  • Timely access to care
  • Have treatment risks, options, and procedures clearly explained in terms you can understand
  • Decline treatment if you choose and be informed of the potential consequences of that decision
  • Request and use an interpreter during appointments when needed
  • File a complaint without fear of discrimination and have it taken seriously and addressed

As a Mountain View Medical Center Patient, you also have the Responsibility to:

  •  Be respectful to your health care provider and staff
  • Keep your appointments or cancel at least 24 hours ahead of time if you can’t make it
  • Share honest and complete information about your health so you can get the best care
  • Let your clinic provider know if you are seeing other health care providers. (e.g.: specialists, behaviorists etc.)
  • Tell your provider about any changes to your medications, including those given by other providers
  • Tell the office staff if your insurance changes
  • Let the clinic know if your phone number or address changes

The Mountain View Medical Center’s Administrative Team or Operations Director will assist any patient who may have questions, complaints or concerns that cannot be resolved through existing processes. These concerns can be addressed by calling 503-359-4773.
